1. Website terms of use

Use of empathyinsuranceadvisors.com constitutes acceptance of these terms. Content on this site is provided for informational and educational purposes. We may update site content at any time without notice.

You may not use the site to harvest contact information, automate scraping, attempt to interfere with site operations, or violate any applicable law. Logos, trademarks, and brand assets on this site are the property of their respective owners (see Section 15).

Use of our site is also subject to our Privacy Policy.

2. Insurance brokerage and license

Empathy Healthcare Solutions Inc. (doing business as Empathy Insurance Advisors) is an independent licensed insurance brokerage. National Producer Number 19291077. Licensed in 22+ states with Federally Facilitated Marketplace certification.

License details are verifiable on the National Insurance Producer Registry. Specific state license numbers and carrier appointments are available on request.

This is a solicitation for insurance.

3. Government affiliation

We are not connected to or endorsed by the U.S. government or the federal Medicare program. Information about Medicare on this site is provided for educational purposes. Official Medicare information is available at Medicare.gov, by calling 1-800-MEDICARE (1-800-633-4227), or through your local State Health Insurance Assistance Program (SHIP).

4. Medicare TPMO disclaimer

As a Third Party Marketing Organization (TPMO) for Medicare under Centers for Medicare & Medicaid Services (CMS) regulations, we provide the following disclaimer required by 42 CFR §§ 422.2267(e)(41) and 423.2267(e)(41):

We do not offer every plan available in your area. Currently we represent [X] organizations which offer [Y] plan options in your area. Please contact Medicare.gov, 1-800-MEDICARE, or your local State Health Insurance Assistance Program (SHIP) to get information on all of your options.

5. Medicare educational content

Medicare-related content on this site is educational in nature, intended to help you understand the structure of Medicare and the trade-offs among coverage options. We do not steer toward any specific carrier or plan.

Specific Medicare Advantage and Part D plan benefits are addressed during a personal consultation, in which a Scope of Appointment is completed in advance per CMS rules where applicable. Plan benefits, premiums, and cost-sharing change annually; current information is confirmed at the time of enrollment.

6. ACA Marketplace disclosures

ACA Marketplace coverage is administered by the U.S. Department of Health & Human Services through HealthCare.gov in Federally Facilitated Marketplace states. Some states operate their own state-based exchanges.

Eligibility for premium tax credits and Cost-Sharing Reductions is based on household income, household size, and other factors per IRS and HHS rules. Subsidy amounts are estimates until reconciled at tax time. Actual amounts depend on actual income for the coverage year.

Plan availability, premiums, and benefit structures vary by carrier, county, and plan year. Information presented during consultation is based on current Marketplace offerings at the time of consultation.

7. Short-Term Medical (STM) disclosure

The following federal disclosure applies to all Short-Term Medical / Short-Term Limited-Duration Insurance products discussed or marketed by us. The text appears verbatim per the federal operative model:

THIS COVERAGE IS NOT REQUIRED TO COMPLY WITH CERTAIN FEDERAL MARKET REQUIREMENTS FOR HEALTH INSURANCE, PRINCIPALLY THOSE CONTAINED IN THE AFFORDABLE CARE ACT. Be sure to check your policy carefully to make sure you are aware of any exclusions or limitations regarding coverage of preexisting conditions or health benefits (such as hospitalization, emergency services, maternity care, preventive care, prescription drugs, and mental health and substance use disorder services). Your policy might also have lifetime and/or annual dollar limits on health benefits. If this coverage expires or you lose eligibility for this coverage, you might have to wait until an open enrollment period to get other health insurance coverage.

Short-Term Medical is not minimum essential coverage and does not satisfy the federal definition of individual health insurance coverage. STM termination does not trigger an ACA Special Enrollment Period. Plan availability, term length, and renewal rules vary by state. See the Private Insurance page for additional context.

8. Limited Medical / Fixed Indemnity disclosure

The following federal disclosure applies to all Limited Medical / Fixed Indemnity excepted-benefit products discussed or marketed by us in the individual market. The text appears verbatim per the 2024 Final Rule:

IMPORTANT: This is a fixed indemnity policy, NOT health insurance. This fixed indemnity policy may pay you a limited dollar amount if you're sick or hospitalized. You're still responsible for paying the cost of your care. Looking for comprehensive health insurance? Visit HealthCare.gov or call 1-800-318-2596 (TTY: 1-855-889-4325). To find out if you can get health insurance through your job, or a family member's job, contact the employer. Questions about this policy? Contact your State Department of Insurance. Find their number on the National Association of Insurance Commissioners' website (naic.org).

9. Life insurance — NAIC Model 570 disclosures

Life insurance content on this site complies with the National Association of Insurance Commissioners (NAIC) Model Regulation 570 governing advertisements of life insurance and annuities, plus applicable state Department of Insurance advertising rules.

Life insurance is not described or marketed as an investment, savings plan, retirement plan, profit- sharing arrangement, deposit, or private pension. These terms are reserved for products that are substantively different from life insurance and that are regulated under different frameworks.

Insurance products are subject to underwriting; rates and availability vary by health, age, state, and carrier. Sample rates referenced during a consultation are preliminary estimates only and are not guarantees of actual policy terms. Final premiums are determined by the carrier upon underwriting completion.

Final Expense (simplified-issue whole life) policies are subject to senior consumer protection rules in adopting states (including California, Florida, Texas, and Ohio). Phone-recording disclosure is provided when collecting senior leads by phone where required.

10. Replacement transaction disclosure (life insurance)

NAIC Model Regulation 26 governs the replacement of existing life insurance policies. If we recommend or facilitate the replacement of an existing life insurance policy with new coverage, applicable replacement transaction disclosures, comparative summaries, and acknowledgement forms required by your state will be completed at the point of application.

Replacing existing life insurance is a serious decision with potential financial consequences. We do not recommend replacement that does not clearly improve your coverage or your situation, and we follow the documentation requirements imposed by NAIC Model 26 and your state Department of Insurance.

11. Educational content disclaimer

Content on this site is for educational purposes. It is not a substitute for professional advice from a licensed insurance advisor familiar with your specific situation, a tax professional, an attorney, or a healthcare provider. Information is believed accurate at the time of publication but regulations, plans, and pricing change.

12. No warranty

13. Carrier independence

We are an independent licensed insurance brokerage. We are not employed by, controlled by, or owned by any single insurance carrier. We are appointed with multiple carriers in each state where we are licensed and earn commission from carriers when clients enroll in coverage. Commission rates are set by carriers and are similar across competing products in most cases.

Carrier names referenced on this site are referenced in plain text only. Carrier logos are not displayed without explicit written permission from the carrier.

14. Lead generation disclosure

Information you submit through consultation request forms, phone calls, or other inbound contact is used by Empathy Insurance Advisors licensed advisors to evaluate coverage options and respond to your inquiry. We do not sell your information to third-party lead aggregators.

Carrier names and product references appearing on this site are trademarks of their respective owners and are used for identification purposes only. Their appearance does not imply sponsorship or endorsement.

16. Scope of practice

Empathy Insurance Advisors is not a real estate, tax, or financial planning firm. We provide licensed insurance brokerage services within the verticals we represent. For tax planning, financial planning, legal advice, or real estate matters, consult an appropriately licensed professional.

17. Modifications to these terms

We may update these terms and disclaimers from time to time. The “Last updated” date at the top of this page reflects the most recent revision. Material changes will be highlighted in a notice on the site.
